All too often we hear our clients say that they are finding it more and more difficult to provide timely care to their office patients if they happen to become hospitalized. After all, running a successful outpatient practice is quite demanding, and finding the time to make rounds in the hospital can be a real task. Despite this general consensus amongst PCP’s, most are uneasy and unwilling to hand their hospitalized patients over to an unfamiliar hospitalist group. Why? Well… Over the past couple of years, there has been a growing disconnect between the inpatient and outpatient world. Unfortunately, it has become a recurrent occurrence for patients to show up to their primary care physician’s office weeks or even months after a major medical occurrence, leaving both parties asking the same question, “How come they didn’t tell you about your hospitalization?”
At Accountable Inpatient Medicine, we aim to bridge the gap between the inpatient and outpatient world. Our Transition of Care Coordinator works diligently with our clients to assure that they are notified when their patients are hospitalized and have adequate follow-up by the time of discharge. Our practice is so tightly knit that we often hand out our personal cellphone numbers to clients assuring proper communication and availability at all times.
